Minor Tattoo Consent & Release Form

At Salvation Tattoo Lounge, we welcome minors for tattoos; however, all clients under the age of 18 must follow strict guidelines. Before your appointment, you must print out our Minor Release Form, have it completed and signed by a parent or legal guardian, and bring it with you.

To ensure compliance with local regulations, both the minor and their guardian must be present at the appointment with a valid government-issued ID. Please note that certain designs or placements may not be available for minors, as our artists prioritize safety, professionalism, and long-term suitability.

We encourage all minors to consider the permanence of tattoos, potential skin sensitivity, and aftercare requirements before making a decision. If you have any questions, please contact us in advance.
